Consent to the processing of personal data
I consent to the processing of my personal data provided during the account registration by Atman sp. z o.o. - the data controller - in order to provide the services specified in the order form.*
Atman sp. z o.o. declares that for the purpose of performing the agreement, it processes personal data such as: first and last names, as well as e-mail addresses and telephone numbers of the persons indicated in the order form filled out for the purpose of concluding an agreement and providing services (hereinafter referred to as “Personal Data”): Personal Data, in accordance with the provisions of the Regulation of the European Parliament and of the Council (EU) 2016/679 of April 27, 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and the repeal of Directive 95/46/EC (General Data Regulation) (hereinafter: “GDPR”). Atman further declares that: (i) it will process personal data for the duration of the agreement and for the purposes of its execution pursuant to Art. 6 (1) lit. b GDPR; (ii) it has obligated its employees, affiliates or subcontractors who have access to Personal Data to keep such data confidential and to comply with the data protection mechanisms provided for under the GDPR, (iii) it has taken organizational and technical measures to secure the Personal Data, (iv) it will, where possible, return to the Customer or destroy the Personal Data immediately upon termination or expiration of the Agreement, unless applicable law requires Atman to retain such data for a longer period, (v) promptly notify Customer of any security breach affecting Personal Data and assist Customer in remedying the effects of the breach, (vi) if necessary, assist Customer in responding to individual requests from individuals exercising their rights under the GDPR.

Acceptance of terms and conditions
I declare that I have read the provisions of the terms and conditions (together with the appendices: price list , technical parameters and SLA and software terms of use ) and hereby accept the contents of the aforementioned documents.

I declare that I run a sole proprietorship and I request that Atman sp. z o.o. deliver and start providing the ordered services before the end of the legal 14-day period for withdrawing from a distance contract, and I therefore waive the possibility of exercising this right with respect to the services ordered by me.
